Change of Membership Information
We will be happy to update your records if you have
moved, changed your name, phone number or email address or
have some other change that you would like to make in your Guild membership
registration. Please provide us with
the "old" information so we will be sure we are changing your record and not
someone else's! Simply fill out the
form below (only the fields that are changing - old and new) and click Submit. This will send an email to the
Membership Chairperson and your changes will be
recorded. Thanks!
